Move on solutions for people with additional health and care needs

Finding suitable move on options from temporary accommodation comes with challenges, but when a client has additional health or care needs the requirements become more specific and the importance of finding the right home for the right person is highlighted.

In this session we will hear from three speakers each providing their own case-study in move-on solutions for people with additional health and care needs. We will cover subjects including:

• the additional support needs of the groups they are working with and when the need for separate move-on solutions became apparent
• what is involved in the running of care-homes and the funding models
• benefits and impact of the provision and a person centred approach to finding a home that fits the unique needs of the client
